Direct Service Provider/HHA Job Description:

Supervisor (LPN/Nurse) Job Description:
- Shall complete and document initial home visit at the start of care
- Define the expected activities of the homemaker and prepare a written plan consistent with the PAA service checklist that has been completed by the PAA and the enrollee prior to client service initiation
- Shall continue to evaluate homemaker’s compliance with the activity plan, and HMK performance
- The supervisor shall discuss recommended modifications with the PAA
- Conduct and document a visit to the enrollee at least every ninety days to evaluate the Homemaker’s compliance with the plan of care
- Conduct and document a supervising visit to the client at least 60 days while the Homemaker is present and providing enrollees’
